Answer:
A police officer in Forest Hills, PA, was shot and seriously injured in the summer of 2003 when his Second Chance Ultima® vest made of Zylon® was penetrated. The Forest Hills, PA, incident is the first case report to the National Institute of Justice (NIJ) where NIJ-compliant body armor appears to have failed to prevent a bullet from penetrating that it was designed to defeat. NIJ is still reviewing this incident to determine the cause of the armor failure.
