Bank Status: This section may have one
of three different status lines.
1)
We have not received you signed Bank form.- The program has never received
a Bank form from your jurisdiction.
2) We
received your signed Bank form on (date of approval).- The program has received
a proper Bank form which has been verified by the Jurisdiction's bank official
and a business card was attached.
3)
We received your signed Bank form on (original approval date) and requires
revalidation. Please send a signed Bank form with the new information.-
Your Banking information has been changed to reflect new or updated information.
It is necessary to print out a new Bank Form with the corrected information.
Take the corrected form to your bank and have the new form approved by your
bank official. Please include a business card from the approving bank official.
