Abstract
The OFR Data System structure, variables, and responses were developed by the OFR Data Work Group topic leads and members. An OFR team collects information from partners across multiple agencies for the purpose of understanding the circumstances leading to an overdose fatality. The ability of the team to know whether an event or circumstance occurred is limited to the information and records available to the team. The guidance provided on collecting and reporting the data includes instructions intended to increase the reliability and comprehensiveness of the information. Instruction is provided for entering data in each item of the formatted database.
