This American Medical Association questionnaire for corrections officials is intended to document the health services available in jails at two points in time: upon receipt of the questionnaire and after the jail is officially surveyed or a complete health care program has been implemented.
Abstract
The questionnaire includes 169 items and requires 'yes' or 'no' answers. It questions whether the jail has a manual of written policies and defined procedures approved by the health authority, whether the health care delivery is adequate, what health care staff are available, whether certain medical procedures are available, what the level is of inmate participation in health care services, and other concerns.
