Abstract
In 2011, the California State Legislature passed legislation aimed at decreasing the cycling of lower level inmate-patients in and out of State prisons. To ensure that inmates receiving services while incarcerated continue to receive services upon release, the statewide Mental Health Program has coordinated with county departments of mental health (DMH) for continuity of care for these patients. This document contains planning information for use by county DMHs to plan for and implement cost-effective measures to ensure an optimal transition process for inmate-patients requiring mental health services post-release.
