This manual provides the forms to be used by the Miami-Dade Schools Police Department (Florida) and presents the objectives and procedures for completing the forms.
Abstract
The forms have four major purposes in data collection by field personnel. First, they provide a standardized record of the field personnel's activities and findings regarding incidents. Second, the forms provide information that should establish the object of attack; identify the elements of the crime; identify the perpetrators as either known or unknown; identify the victim; may or may not establish motive; identify all persons involved as principals, accessories, victims, or witnesses; and identify all property and investigative leads. A third objective is to ensure that data collected meet all the requirements of all entities of the Department and of the criminal justice system. Fourth, the forms are intended to prevent duplication of the efforts of other field elements required in the follow-up of the initial investigation. The forms are multi-purpose offense reports, in that all incidents are reported by using the same forms. Although the report may consist of several forms, each collects a portion of the information on the total crime. The forms are designed with a combination of forced-choice, fill-in-the-blanks, and narrative formats. The eight forms provided for case reporting are as follows: Offense/Incident Report, Narrative Continuation Report, Person Report, Property Report, Vehicle Report, Vehicle/Property Report, Multiple Suspect Report, and Multiple Juvenile Suspect Report.
