Abstract
This document contains seven questionnaires that were developed for use in the follow-up phase (waves 4 through 7) of the National Survey of Parents and Youth. The first four questionnaires are screening questionnaires designed to be administered primarily by telephone. These questionnaires include the General Household Screener, introducing the study and verifying the address of the household; the Dyad Screener used for determining the current living arrangements, location, and status of youth and parents selected in wave 1; the Determine Eligibility program used to determines if the selected youth was eligible to participate in the follow-up and if the selected parent was still eligible; and the New Parent program used to allow selection of a new parent for follow-up if the other parent was deemed ineligible. The four screening questionnaires are followed by three substantive questionnaires: one for children aged 9 to 11; one for adolescents and teens aged 12 to 18; and one for parents. These questionnaires obtained information from the respondents that covered, among other topics, basic demographics, school and religion, media consumption, personal use of alcohol, cigarettes, and illicit drugs, communication with parent/child, participation in drug education/classes, and personal participation in community drug prevention activities.
