Abstract
This document contains the survey questions for the Native American Substance Abuse Treatment Needs Study. The survey will be used to obtain more substance abuse treatment services. Some of the questions asked were: general health questions, cigarette use, smokeless tobacco use, and use of alcohol. Additions questions include drug use of the following: marijuana, hallucinogens, cocaine/crack, heroin or other opiates/opioids, sedatives, stimulants/speed and inhalants, and treatment history. The survey questions are followed by the frequency tables.
